Below you will find the only two options available for individual and family health insurance coverage with Wellmark; traditional major medical and health savings accounts. The traditional major medical plans provide three choices for coverage; Essential, Enhanced and Comprehensive. The health savings account plans do not cover maternity or mental health. After reviewing the plans, look up your monthly premium. Make sure to look for the tobacco and non-tobacco rates. Complete the application and mail to the application processing center below. All applications are processed the day they are received. Do not mail money with your application. You will be billed monthly upon acceptance.
